Privacy policy


This privacy statement covers the processing of personal data for website visitors and clients. We respect the privacy of our clients and handle personal data with care. We process personal data only for the purpose for which it was provided. We adhere to the requirements of the General Data Protection Regulation (GDPR). This privacy statement informs you of how we may process your personal data. This privacy statement was last changed on 11 November 2021.

For what purposes do we process which personal data?
We (Quadrivas Therapists) process personal data for various purposes, which are explained in more detail below. Not all of these processing purposes will apply to you. If a processing purpose and/or any part of it does not apply to you, then your personal data mentioned below will also not be processed for that purpose. 

Purpose: Information (if you have given consent e.g. via social media, e-mail, the website). To inform you about treatments and send newsletters: your name, e-mail address and/or telephone number. 

Purpose: Treatment (basis for processing is the treatment agreement). To treat you as a client: your name, address, date of birth, e-mail address, telephone number, health data for the treatment agreement, a client file and financial data.

Privacy of client file
Proper treatment requires your treating Quadrivas Therapist to keep a file. Your file contains notes and details of the Quadrivas treatments performed. We are committed to ensuring your privacy. This means, among other things, that we handle your personal data carefully, and ensure that unauthorised persons cannot access your data. The treating therapist has sole access to the data in your file. There is a legal duty of confidentiality (professional secrecy).

How is your personal data collected?
To best protect your personal data, the Quadrivas Therapist takes appropriate administrative and technical measures to prevent misuse, loss, unauthorised access, unwanted disclosure, unauthorised modification or otherwise unlawful processing. Only she has access to your personal data.

What rights do you have and how can you exercise them?
The GDPR gives individuals several rights to safeguard their privacy: Right to access your personal data; Right to correct/add to your personal data; Right to delete your personal data; Right to restrict processing of your personal data; Right to object to processing of your personal data. The request to exercise one or more of the above rights can be made in writing via the e-mail address of your treating Quadrivas Therapist, after which you will receive a confirmation within 10 business days. If you have a complaint about the processing of your personal data, please let us know. If we cannot find a solution together, we of course find this very unfortunate. You always have the right to file a complaint with the Dutch Data Protection Authority.

How long is your personal data stored?
Personal data will not be kept any longer than necessary for the purposes for which it was collected. The WGBO (Medical Treatment Agreement Act) recommends a retention period of at least 5 years. 5 years after the last treatment, your data will be deleted unless you decide otherwise.

Transmission of personal data to third parties
Your personal data will not be provided to third parties unsolicited. Example: if, when referring to a doctor or physiotherapist, this person/agency needs data, we will only do so at your request and only with your explicit consent. However, if there is a request where we are legally obliged to provide information, we will do so. We do not provide personal data to parties based outside the EU.

Cookies
Functional and analytical (Google Analytics) cookies are collected. You can adjust your browser's cookie management yourself by adjusting its settings (this may affect the functioning of the sites you then view).

Third-party websites
This statement does not apply to third-party websites that are connected to this website through links. We cannot guarantee that these third parties handle your personal data in a reliable or safe manner. We recommend that you read the privacy statements of these websites before using them.

Privacy statement changes
We reserve the right to make changes to this statement. It is recommended to regularly consult this statement so that you are aware of these changes.

Accessing and correcting your data
